Registration Form – Summer Program
Completed forms can be emailed to northbeachschoolofdance@comcast.net, mailed to PO Box 578, North Beach MD 20714, or brought in person during office hours (Mon-Thurs, 6-8 pm; Sat 9:30-10:30 am).
The registration fee can be paid in person or over the phone at the office during office hours. A check can be mailed to P.O. box 578, North Beach MD 20714. Payment can also be made online through our website. Any payments made online incur a $3 convenience fee. It is the responsibility of the payer to add the fee to the total.
